Instead, Americans are forced to rely on a mixture of profit and nonprofit private and public healthcare insurers and providers. The United States federal government <a href=\"https:\/\/www.commonwealthfund.org\/international-health-policy-center\/countries\/united-states\" target=\"_blank\" rel=\"noreferrer noopener\">provides<\/a> healthcare coverage through Medicare to individuals ages 65 years and older, and to some individuals with disabilities, military veterans, and children through Medicaid and the Children\u2019s Health Insurance Program (CHIP).\u00a0<\/p>\n<p><a href=\"https:\/\/www.commonwealthfund.org\/international-health-policy-center\/countries\/united-states\" target=\"_blank\" rel=\"noreferrer noopener\">Around 26 million Americans<\/a>, about 8% of the population, including just under 2% of children, have no health insurance coverage at all. Low-income families are more likely to be uninsured, with the high cost of health insurance cited as the <a href=\"https:\/\/www.kff.org\/uninsured\/issue-brief\/key-facts-about-the-uninsured-population\/\" target=\"_blank\" rel=\"noreferrer noopener\">main factor<\/a> as to why people remain uninsured in the US. The lack of coverage significantly worsens Americans\u2019 access to health care and many face unaffordable out-of-pocket medical bills if they do seek care.\u00a0<\/p>\n<figure class=\"wp-block-pullquote alignright has-text-align-left\">\n<blockquote>\n<p>A study published in 2019 <a href=\"https:\/\/ajph.aphapublications.org\/doi\/10.2105\/AJPH.2018.304901?eType=EmailBlastContent&amp;eId=a5697b7e-8ffc-4373-b9d2-3eb745d9debb&amp;=&amp;\" target=\"_blank\" rel=\"noreferrer noopener\">found 530,000 bankruptcies<\/a> filed by individuals every year are due, at least in part, to medical debt.<\/p>\n<\/blockquote>\n<\/figure>\n<p>A 2009 <a href=\"http:\/\/www.pnhp.org\/excessdeaths\/health-insurance-and-mortality-in-US-adults.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> by Harvard Medical researchers found that 45,000 Americans die every year directly as a result of not having health insurance. <a href=\"https:\/\/www.cnn.com\/2019\/11\/12\/health\/us-cant-afford-health-care-trnd\">13<\/a>% of Americans, about 34 million people, reported in a 2019 Gallup poll that they had a friend or family member pass away prematurely after being unable to afford medical care.\u00a0<\/p>\n<p>The uninsured rate has declined since the COVID-19 pandemic as a <a href=\"https:\/\/www.asha.org\/news\/2022\/federal-public-health-emergency-updates-for-2022\/#:~:text=The%20PHE%20has%20been%20in,notice%20before%20the%20PHE%20ends.\" target=\"_blank\" rel=\"noreferrer noopener\">Federal Public Health Emergency<\/a> (in place until at least January 2023) has laxed continued enrollment eligibility for individuals with Medicaid. It\u2019s <a href=\"https:\/\/www.theguardian.com\/us-news\/2022\/may\/26\/millions-risk-losing-us-healthcare-when-pandemic-emergency-declaration-expires\" target=\"_blank\" rel=\"noreferrer noopener\">estimated that anywhere from 5.3 million to 14.2 million Americans<\/a> could lose Medicaid coverage when the federal public health emergency is lifted.\u00a0<\/p>\n<p><a href=\"https:\/\/www.sanders.senate.gov\/press-releases\/news-sanders-introduces-medicare-for-all-with-14-colleagues-in-the-senate\/\" target=\"_blank\" rel=\"noreferrer noopener\">Nearly an additional 50 million Americans<\/a> are underinsured, as many Americans with health insurance have to pay fees and copays on medical treatment ranging from drug prescriptions to doctor visits, or pay deductibles\u2014out-of-pocket costs that must be reached before a health insurer begins to cover medical costs. A 2018 <a href=\"https:\/\/www.norc.org\/NewsEventsPublications\/PressReleases\/Pages\/new-survey-reveals-57-percent-of-americans-have-been-surprised-by-a-medical-bill.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">survey<\/a> conducted by NORC at the University of Chicago found 57% of Americans have been surprised by a medical bill they initially thought was covered under their health insurance, often due to a doctor being out of their health insurer\u2019s network or a product or test not being covered under their health insurance plan.\u00a0<\/p>\n<p><a href=\"https:\/\/www.kff.org\/report-section\/ehbs-2021-summary-of-findings\/\" target=\"_blank\" rel=\"noreferrer noopener\">Around 155 million non-elderly Americans<\/a> rely on health insurance coverage provided through their employer, with the average annual premiums in 2021 for employer-sponsored health insurance sitting at $7,739 for single individual coverage and $22,221 for family coverage. On average, 17% of costs for individual coverage and 28% of the costs for family coverage come out of workers\u2019 own income.\u00a0<\/p>\n<p>When an individual loses their job, they must find new healthcare coverage or sign up for a federal program called the Consolidated Omnibus Budget Reconciliation Act (COBRA), to extend their healthcare plan up to 18 months. However, workers must pay the full costs and an administrative fee to do so, which can be prohibitively expensive. \u2018<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6711468\/\" target=\"_blank\" rel=\"noreferrer noopener\">Job lock<\/a>\u2019 is a term referring to workers feeling compelled to remain in an undesired job for fear of losing income or benefits such as healthcare coverage, and the number of employers offering health insurance coverage has been <a href=\"https:\/\/www.niskanencenter.org\/whats-wrong-with-employer-sponsored-health-insurance\/\" target=\"_blank\" rel=\"noreferrer noopener\">declining<\/a> as costs rise for both employers and workers.<\/p>\n<p>Despite the lack of universal healthcare coverage in the US, the country spends significantly more on healthcare related costs than comparable countries. In 2021, the US spent an <a href=\"https:\/\/www.pgpf.org\/blog\/2022\/07\/how-does-the-us-healthcare-system-compare-to-other-countries\" target=\"_blank\" rel=\"noreferrer noopener\">estimated $12,318 per person<\/a> on healthcare, the highest per capita compared to other wealthy nations in the Organisation for Economic Co-operation and Development (OECD). Excluding the US, the average for wealthy OECD countries was $5,829 per person, with the UK spending $5,387 per person.\u00a0<\/p>\n<p>$1,055 per person in the US was spent solely on administrative costs, compared to $97 per person in the UK. A 2019 <a href=\"https:\/\/www.americanprogress.org\/article\/excess-administrative-costs-burden-u-s-health-care-system\/\" target=\"_blank\" rel=\"noreferrer noopener\">analysis<\/a> by the Center for American Progress estimated the US spends\u00a0 $248 billion on excess healthcare administrative costs annually. The significant healthcare spending costs in the US do not translate to better health outcomes; <a href=\"https:\/\/www.cdc.gov\/nchs\/pressroom\/nchs_press_releases\/2022\/20220831.htm\" target=\"_blank\" rel=\"noreferrer noopener\">life expectancy<\/a> in the US in 2021 was 76.1 years and the US <a href=\"https:\/\/www.commonwealthfund.org\/publications\/fund-reports\/2017\/jul\/mirror-mirror-2017-international-comparison-reflects-flaws-and\" target=\"_blank\" rel=\"noreferrer noopener\">ranks<\/a> behind other wealthy countries in performance metrics for health care outcomes. The US spends less per person on long-term healthcare costs than the average for OECD countries.<\/p>\n<figure class=\"wp-block-pullquote alignright\">\n<blockquote>\n<p>Medical costs are <a href=\"https:\/\/www.fiercehealthcare.com\/finance\/medical-debt-now-outweighs-all-other-personal-debt-us-those-medicaid-nonexpansion-states\">the largest contributor<\/a> to personal debt in the US, surpassing all other debt in collections combined, and hitting individuals in states that have declined to expand Medicaid, people in low-income communities, and Black Americans the hardest.\u00a0<\/p>\n<\/blockquote>\n<\/figure>\n<p>A 2022 <a href=\"https:\/\/www.healthsystemtracker.org\/brief\/the-burden-of-medical-debt-in-the-united-states\/#:~:text=We%20find%20that%2023%20million,%24195%20billion%20in%20medical%20debt.\" target=\"_blank\" rel=\"noreferrer noopener\">analysis<\/a> by the Kaiser Family Foundation found 23 million Americans, 1 in 10 US adults, have significant debt from medical expenses, at least $250, owing a total of at least $195 billion in medical bills. Middle-aged adults and Black Americans are most likely to hold some form of medical debt. In a 2022 <a href=\"https:\/\/www.kff.org\/health-costs\/issue-brief\/americans-challenges-with-health-care-costs\/\" target=\"_blank\" rel=\"noreferrer noopener\">poll,<\/a> the Kaiser Family Foundation found that 4 in 10 US adults, or 41%, reported having medical or dental debt, which includes debt owed on credit cards, collection agencies, family, friends, banks, or other lenders. Thousands of Americans are forced to crowdfund for assistance with high medical bills and costs related to debilitating illnesses such as <a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2022\/07\/09\/1110370391\/cost-cancer-treatment-medical-debt\" target=\"_blank\" rel=\"noreferrer noopener\">cancer<\/a>. The online fundraising organization <a href=\"https:\/\/www.gofundme.com\/start\/medical-fundraising\" target=\"_blank\" rel=\"noreferrer noopener\">GoFundMe<\/a> claims over 250,000 fundraisers started by individuals or organizations are started every year, raising more than $650 million per year. The <a href=\"https:\/\/www.usnews.com\/news\/health-news\/articles\/2022-02-04\/crowdfunding-for-medical-costs-almost-always-fails\" target=\"_blank\" rel=\"noreferrer noopener\">majority<\/a> of crowdfunding campaigns for medical expenses fail, with research showing almost 90% do not reach the set goals and only half reach 25%of the set fundraising goal, and campaigns in the highest income ZIP codes raised significantly more funds.\u00a0<\/p>\n<p>Americans pay higher prices for prescription drugs than any other country in the world, at rates <a href=\"https:\/\/aspe.hhs.gov\/reports\/trends-prescription-drug-spending#:~:text=Americans%20pay%20higher%20prices%20for,other%20similar%20high%2Dincome%20nations.\" target=\"_blank\" rel=\"noreferrer noopener\">2.5 times<\/a> as high as prices in similar-income nations. According to a <a href=\"https:\/\/www.kff.org\/health-reform\/poll-finding\/kff-health-tracking-poll-february-2019-prescription-drugs\/\" target=\"_blank\" rel=\"noreferrer noopener\">poll<\/a> conducted by the Kaiser Family Foundation in February 2019, 24% of adults and 23% of senior citizens reported difficulty in affording their prescription medications. A Gallup poll published in November 2019 <a href=\"https:\/\/thehill.com\/policy\/healthcare\/470031-poll-1-in-5-us-adults-report-trouble-affording-prescription-drugs\" target=\"_blank\" rel=\"noreferrer noopener\">reported<\/a> 22.9% of Americans could not afford their prescription medications at least once over the past year. The discrepancies of prescription drug prices are so high, Americans who live near the borders of <a href=\"https:\/\/www.bbc.com\/news\/av\/world-us-canada-49350033\" target=\"_blank\" rel=\"noreferrer noopener\">Canada<\/a> or <a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2019\/02\/11\/691467587\/americans-seek-cheaper-meds-in-mexico\" target=\"_blank\" rel=\"noreferrer noopener\">Mexico<\/a> frequently take trips over the border just to buy prescription drugs because the savings for the same exact drugs are so substantial. These high costs often result in Americans filing for bankruptcy due to the burden of medical debt. A study published in 2019 <a href=\"https:\/\/ajph.aphapublications.org\/doi\/10.2105\/AJPH.2018.304901?eType=EmailBlastContent&amp;eId=a5697b7e-8ffc-4373-b9d2-3eb745d9debb&amp;=&amp;\" target=\"_blank\" rel=\"noreferrer noopener\">found 530,000 bankruptcies<\/a> filed by individuals every year are due, at least in part, to medical debt.\u00a0<\/p>\n<p>As debt for individuals continues to pile up, hospitals go out of their way to collect, even suing families for payment. A recent <a href=\"https:\/\/khn.org\/news\/article\/medical-debt-hospitals-sue-patients-threaten-credit-khn-investigation\/\" target=\"_blank\" rel=\"noreferrer noopener\">investigation <\/a>by Kaiser Health News of more than 500 different hospitals in the US found at least two-thirds of hospitals sue patients over medical bills, including legal actions such as garnishing wages or placing liens on their property. A quarter of hospitals sell patients\u2019 debts to debt collectors and about 1 in 5 hospitals deny non-emergency medical care to patients over past-due medical debt.\u00a0<\/p>\n<p>Medical costs are <a href=\"https:\/\/www.fiercehealthcare.com\/finance\/medical-debt-now-outweighs-all-other-personal-debt-us-those-medicaid-nonexpansion-states\" target=\"_blank\" rel=\"noreferrer noopener\">the largest contributor<\/a> to personal debt in the US, surpassing all other debt in collections combined, and hitting individuals in states that have declined to expand Medicaid, people in low-income communities, and Black Americans the hardest.\u00a0<\/p>\n<p>In many cases, Americans either ration medication or delay medical treatment because of the high costs. For example, an estimated 1.3 million out of the <a href=\"https:\/\/apnews.com\/article\/health-prices-diabetes-congress-a2f9986b7bf3500b81d1ec80a01e5abb#:~:text=Roughly%208.4%20million%20Americans%20use,do%2C%20it's%20an%20important%20medication.\" target=\"_blank\" rel=\"noreferrer noopener\">8.4 million Americans<\/a> who rely on injections of insulin to control their diabetes were forced to ration their medication due to the <a href=\"https:\/\/www.rand.org\/blog\/rand-review\/2021\/01\/the-astronomical-price-of-insulin-hurts-american-families.html\" target=\"_blank\" rel=\"noreferrer noopener\">exorbitantly high prices set by pharmaceutical companies.<\/a> Numerous stories have been reported in the media in recent years of families losing loved ones after they were forced to ration insulin because they couldn\u2019t afford to keep up with the costs of it.<\/p>\n<figure class=\"wp-block-pullquote alignright has-text-align-left\">\n<blockquote>\n<p>All of the harrowing statistics, economics, and moral failings of the American healthcare system provide numerous reasons for why the US needs universal healthcare.<\/p>\n<\/blockquote>\n<\/figure>\n<p>Additionally, a <a href=\"https:\/\/news.gallup.com\/poll\/269138\/americans-delaying-medical-treatment-due-cost.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">poll<\/a> conducted by Gallup in 2019 found 25% of Americans reported they or a family member delayed treatment for a serious medical condition due to the potential high cost, and an additional 8% said they or a family member delayed medical treatment for a less serious condition due to cost.\u00a0<\/p>\n<p>Further compounding the problem, the US also <a href=\"https:\/\/www.kff.org\/womens-health-policy\/fact-sheet\/paid-leave-in-u-s\/#:~:text=The%20federal%20Family%20and%20Medical,leave%2C%20despite%20strong%20public%20support.\" target=\"_blank\" rel=\"noreferrer noopener\">does not mandate<\/a> paid sick or family leave, while nearly all other industrialized nations do so. <a href=\"https:\/\/www.abetterbalance.org\/sick-without-a-safety-net\/#:~:text=The%20United%20States%20is%20the,paid%20sick%20leave%20for%20workers.&amp;text=Because%20of%20this%20major%20gap,a%20single%20paid%20sick%20day.\" target=\"_blank\" rel=\"noreferrer noopener\">Over 33 million American workers<\/a> do not have a single paid sick day.\u00a0<\/p>\n<p>Numerous news reports in the US in recent years have exposed harrowing instances of exorbitant medical bills, costs, and unethical business practices rampant in the US healthcare industry. During the COVID-19 pandemic, several individuals <a href=\"https:\/\/www.nytimes.com\/2021\/05\/21\/upshot\/covid-bills-financial-long-haulers.html\" target=\"_blank\" rel=\"noreferrer noopener\">faced<\/a> thousands of dollars in medical bills for treatment, despite legislative efforts to mitigate or eliminate costs for COVID-19 care. A 2022 Gallup <a href=\"https:\/\/news.gallup.com\/poll\/393494\/older-adults-sacrificing-basic-needs-due-healthcare-costs.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">poll<\/a> found 1 in 3 Americans ages 50 and older have forgone food to be able to pay for healthcare; 37% of Americans ages 65 and older who qualify for Medicare and 45% of adults ages 50-64 reported concern they would not be able to afford needed healthcare services in the next year.\u00a0<\/p>\n<p>US health insurers <a href=\"https:\/\/www.theguardian.com\/us-news\/2020\/aug\/14\/us-health-insurers-coronavirus-pandemic-profit\" target=\"_blank\" rel=\"noreferrer noopener\">reported<\/a> record profits shortly after the onset of the pandemic and the largest US health providers have <a href=\"https:\/\/www.benefitspro.com\/2022\/03\/16\/infographic-breaking-down-health-insurers-2021-performance\/?slreturn=20221006153334\" target=\"_blank\" rel=\"noreferrer noopener\">reported<\/a> billions of dollars in profits, while healthcare workers have <a href=\"https:\/\/www.theguardian.com\/society\/2021\/jul\/30\/us-nurses-strike-covid-coronavirus-conditions-understaffing\" target=\"_blank\" rel=\"noreferrer noopener\">reported <\/a>brutal working conditions during COVID-19 outbreaks, deaths of healthcare workers, and understaffing. A 2022 <a href=\"https:\/\/www.theguardian.com\/us-news\/2022\/jun\/16\/us-could-have-saved-338000-lives-covid-universal-healthcare-study-finds\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> authored by a researcher at the Yale School of Public Health estimated that universal healthcare in the US would have saved more than 338,000 lives and $105 billion in healthcare costs during the COVID-19 pandemic. The United States surpassed <a href=\"https:\/\/coronavirus.jhu.edu\/from-our-experts\/u-s-officially-surpasses-1-million-covid-19-deaths\" target=\"_blank\" rel=\"noreferrer noopener\">1 million reported<\/a> COVID-19 related deaths in May 2022.\u00a0<\/p>\n<p>All of the harrowing statistics, economics, and moral failings of the American healthcare system provide numerous reasons for why the US needs universal healthcare. Medical debt shouldn\u2019t exist and the consequences of gatekeeping healthcare through economic barriers continues to have devastating consequences for American communities, families, and individuals, and is detrimental to public health.\u00a0<\/p>\n<p>This need has been popularized among progressives under calls for <a href=\"https:\/\/www.youtube.com\/watch?v=wwBxWa8ozm4&amp;list=PLstBxrJFQxpC1kK5iGyIfKrkhinpSseqE\" target=\"_blank\" rel=\"noreferrer noopener\">Medicare for All<\/a>, which has received support in polls among Americans by a firm majority across the political spectrum.
